Fahr’s Disease: a case report of a patient with neuropsychiatric symptoms

Introduction Fahr’s disease (FD) is a rare disorder consisting of bilateral and symmetrical calcium deposits in basal ganglia cerebral cortex. These lesions are associated with neurological psychiatric symptoms such as rigid hypokinetic syndrome, mood disorders memory concentration abnormalities. It can be idiopathic or secondary to endocrine disorders, infectious diseases mitochondrial myopathies. Objectives To highlight the importance considering organic causes when evaluating patients presenting atypical claim role neuroimaging. Methods Case report non-systematic review literature: sources obtained from Pubmed database. Results A 69-year-old man, native Syracuse (Italy), was admitted Psychiatry Unit February 2022 behavioural disturbances irritability. In July 2021 he presented same symptoms, being mistakenly diagnosed Bipolar Disease type I. He has no previous history. started changes his personality, short-term loss, aggressiveness disorganized behaviour at age 66. At admission talkative hyperfamiliar, delusions grandiosity, exalted affectivity insomnia. Neurological examination showed problems, signs frontal disinhibition abnormal glabellar tap sign. Blood tests, CT brain MRI were performed rule out underlying causes. Neuro-imaging found symmetric calcifications globus pallidus, thalamus corpus striatum, favour FD. Secondary (abnormalities PTH, vitamin HIV, brucellosis neurosyphilis) where discarded, allowing us conclude it probably primary case Valproate stabilizer anticonvulsant. Genetic tests indicated.
